In Durban, South Africa, from October 22 to 25, 2025, the 4th International Conference on Public Health in Africa (CPHIA 2025) sparked a bold vision for the continent’s health future. Under the theme “Moving Towards Self-Reliance for Universal Health Coverage and Health Security,” over 5,000 delegates—scientists, policymakers, innovators, and global partners—gathered at the Durban International Convention Centre. The highlight? A transformative R5 billion (approximately $280 million USD) pledge to advance African vaccine equity, signaling a shift from aid dependency to homegrown innovation and positioning Africa as a global health leader.

CPHIA 2025: Turning Vision into Action

Co-hosted by the Africa Centres for Disease Control and Prevention (Africa CDC) and the South African government, CPHIA 2025 built on successes from Kigali (2021), Lusaka (2022), and Cairo (2023). This year’s event focused on actionable strategies to tackle climate-driven outbreaks, antimicrobial resistance, and funding gaps. High-level speeches from Africa CDC Director General Dr. Jean Kaseya and South African Deputy Minister of Health Dr. Joe Phaahla called for a “New Public Health Order” driven by African-led solutions.

With over 200 sessions, including plenaries on primary health care and digital health, the conference fostered innovation. Side events highlighted youth leadership through the YES Health initiative and community-driven epidemic responses. Virtual streams engaged thousands globally, amplifying impact. HIV Progress: A Push for Elimination

HIV discussions took center stage, with Africa shouldering 67% of the global burden despite comprising 15% of the world’s population. CPHIA 2025 celebrated milestones like South Africa’s initiation of over 5 million people on antiretroviral therapy (ART). Yet, challenges persist: only 86% of Africans living with HIV know their status, and new infections rose in 12 countries last year.

Key outcomes included a push for domestic financing to close the $10 billion annual funding gap for HIV, TB, and malaria. The Global Fund, represented by Vice Chair Bience Gawanas, pledged support for community-led interventions. Sessions on long-acting PrEP and point-of-care diagnostics highlighted local manufacturing’s potential to cut costs by 50%. Dr. Thembisile Xulu of South Africa’s National AIDS Council urged, “Let’s turn crisis into opportunity, using data and partnerships to end AIDS by 2030.”

Revolutionizing Health Manufacturing

CPHIA 2025 championed local production of vaccines, therapeutics, and diagnostics. Plenary sessions outlined the ecosystem needed: demand forecasting, regulatory alignment, and investment incentives. South Africa’s BioAfrica Convention, integrated into the event, unveiled partnerships to scale monoclonal antibody production for HIV and Ebola, potentially creating 10,000 jobs across Africa.

The R5 billion pledge, backed by African governments, the African Development Bank, and private sector leaders, fuels the African Vaccine Manufacturing Accelerator. This initiative aims to produce 60% of Africa’s vaccine needs by 2030, up from just 1% today. Innovations like mRNA platforms in Rwanda and Senegal promise a $20 billion economic impact. Dr. Nhlanhla Msomi of AfricaBio stated, “Local manufacturing is about sovereignty—saving lives on African terms.”

Local Heroes: South African Innovators

CPHIA 2025 celebrated South Africa’s medical trailblazers. Dr. Salome Maswime, Professor of Global Surgery at the University of Cape Town, shared her AI-driven work on stillbirth data systems, reducing maternal health disparities. “Data is the voice of the voiceless,” she said, “guiding equitable care.”

Dr. Charles Shey Wiysonge, a vaccinologist at Cochrane South Africa, championed evidence-based immunization policies, spotlighting next-generation HIV vaccine trials. Their stories inspired youth forums, showcasing African excellence in action.

Global Partnerships: Sustaining HIV Progress

The U.S. PEPFAR program, having invested $120 billion in Africa since 2003 and saved 25 million lives, faced scrutiny amid funding uncertainties. Delegates proposed a “bridge plan” to sustain services, with South Africa praising PEPFAR’s recent $500 million HIV prevention allocation. Dr. Sania Nishtar of Gavi emphasized, “Africa’s self-reliance builds on PEPFAR’s legacy, ensuring no one is left behind.”

BRICS Boost: Health Tech Alliances

BRICS partnerships, highlighted at CPHIA 2025, advanced health technology transfers. Under the FOCAC Beijing Action Plan (2025-2027), commitments include training 100 African professionals in AI diagnostics and establishing joint medical centers. South Africa’s BRICS leadership fostered investments in telemedicine and genomic surveillance, promising $2 billion in tech infrastructure. Dr. Maha Barakat noted, “BRICS is an alliance empowering Africa to innovate as equals.”

South Africa’s Visionary Leadership

South Africa shone as CPHIA 2025’s host, blending logistical excellence with policy innovation. Minister of Science, Technology, and Innovation Prof. Blade Nzimande linked R&D to economic growth, while eThekwini’s hosting underscored urban health. The R5 billion pledge drives a “health industrial revolution,” creating jobs and curbing brain drain. Deputy Minister Phaahla declared, “South Africa partners with Africa for sovereignty, not charity.”

A Legacy of Resilience

As CPHIA 2025 concluded, its outcome statement—a blueprint for self-reliance—echoes globally. The R5 billion vaccine pledge, paired with HIV progress, manufacturing leaps, and strategic alliances, marks Africa’s ascent as a health innovator. Led by pioneers like Maswime and Wiysonge, and steered by South Africa, the continent nears universal health coverage. Dr. Kaseya’s words ring true: “When Africa stands strong, the world is safer.” The revolution is vibrant, resilient, and unstoppably African.
